4recovery takes every report seriously. If you notice a potential safety risk, privacy issue, or violation of our peer standards, please let us know. Reporting helps us protect everyone who uses this service and maintain alignment with New York Medicaid and CORE safety requirements.

 

Send your concern directly to our restricted Safety Inbox, where it’s reviewed and logged for follow-up. We may reach out for additional details, but we’ll never share your identity outside the safety review process.

When to report

Report whenever you notice a potential safety risk, privacy issue, inappropriate behavior, or violation of our peer standards. Early reporting helps us swiftly address and resolve concerns, maintaining a safe environment for everyone.

What to include

Provide as much context as you can: what happened, when, and where. Include links or screenshots only if it’s safe to do so. Do not attach executable files or medical records.

What happens next

Our Safety team triages within one business day and prioritizes by level of risk. You may be contacted for clarification.

Safety Inbox

Reports go to a restricted mailbox monitored by authorized staff only. Attachments are automatically blocked to prevent data exposure. Records are retained seven years under our compliance policy.
